Customer Self Service Portal Service Request Form

Does anyone know if the service request form in the customer self service portal can be modified to exclude the option for the customer to choose date and time slot?  I would like it if the customer could open a service request here but not have the option to choose preferred time-frame.

    You might try to unset the Moment behaviour called "Choose from different timeslots". It will then show "skip" or "Choose a date" to the customer on the mobile.
